Sagging Neck and Jowls
Why the Neck and Jawline Start to Sag
Sagging jowls and a loose neck are the look of lower-face skin and the tissue under it losing support, so the jawline softens and skin gathers along the chin and throat. It usually shows up in the 40s and 50s, and it has several causes at once: changes in bone and fat under the skin, slower collagen production, years of sun, and gravity. A good routine can improve the look of firmness on the jaw and neck; it cannot rebuild bone or move fat back into place.
What is happening under the skin
- Bone and fat changes. A review of facial aging in Aesthetic Plastic Surgery (Mendelson and Wong, 2012) describes the "prejowl" area of the jaw becoming a relative concavity with age, along with bone loss around the eyes and upper jaw. Less support lets soft tissue fold forward into a jowl.
- Less collagen. In a lab study, skin cells taken from people aged 80 and over made less type I procollagen than cells from people aged 18 to 29 (56 versus 82 ng/ml; Varani and colleagues, 2006).
- Sun exposure. The neck and chest get daily sun and are often skipped with sunscreen. In a 4.5-year trial of 903 adults in Australia, people who used sunscreen daily showed no detectable increase in skin aging on the backs of the hands, and were 24% less likely to show aging than people who used it when they chose to (Hughes and colleagues, 2013).
- Weight changes and genetics. Weight loss leaves skin with less to hold up, and family traits set how early jowls appear.
The At-Home Routine for a Firmer-Looking Jaw and Neck
Think of everything from the jawline to the collarbone as part of your face.
Morning
- Cleanse gently; neck skin is thinner than facial skin.
- Firming serum with peptides and a film-forming tightener, applied with upward strokes.
- Moisturizer so crepey neck skin looks smoother.
- Broad-spectrum sunscreen on the face, neck, chest and backs of the hands, every day.
Evening
- Cleanse.
- Retinol two to three nights a week to start, then more often as your skin adjusts.
- Peptide moisturizer on the other nights, or over the retinol once your skin is used to it.
Ingredients That Help the Look of Sagging
| Ingredient | What it does for the look of the jaw and neck | Usage rate (drops per fl oz) |
|---|---|---|
| Pepha-Tight (algae extract and pullulan) | Forms a light film for an instant tightening feel, plus longer-term firmness in supplier testing | 5% (45 drops) |
| Progeline (trifluoroacetyl tripeptide-2) | Peptide used for slackness along the jaw and neck rather than lines | 2% (18 drops) |
| Relistase (tetrapeptide) | Firming peptide for the jawline and cheeks | 4% (36 drops) |
| Syn-Tacks | Peptide complex for the look of firmer, more supple skin | 3% (27 drops) |
| Matrixyl 3000 | Peptide pair for the look of smoother, better-supported skin | 8% (72 drops) |
| Retinol | Smooths the look of fine lines and crepey texture over months | Use a finished product as directed |
| Vitamin C | Antioxidant support for sun-exposed skin of the neck and chest | Use a finished product as directed |
What the studies measured
- Pepha-Tight. In DSM's supplier testing, 30 volunteers using a gel with 3% Pepha-Tight reported a tightening effect lasting at least 4 hours after one application, and almost 70% rated the tightening good to very good after 2 weeks. In a second test, 12 volunteers used a 3% cream for 56 days; skin tonicity rose 24% and elasticity 15% (supplier data).
- Matrixyl 3000. In Sederma's supplier testing, 23 women (mean age 56) applied a 3% cream to half of the face twice daily for 2 months against placebo. The surface taken up by deep wrinkles fell 39.4%, skin tone improved 15.5% and elasticity improved 5.5% (supplier data).
- Syn-Tacks. In DSM's supplier testing, 15 volunteers aged 40 to 45 used a cream with 1% Syn-Tacks for 56 days. Skin suppleness rose 35%, tonicity 31% and firmness 17% compared with day 0 (supplier data). Skin Perfection uses it at 3%.
- Retinol. In a 24-week double-blind study, 36 elderly adults (mean age 87) applied 0.4% retinol lotion to one arm and a plain lotion to the other. Fine wrinkling improved significantly more on the retinol side (Kafi and colleagues, 2007).
- Vitamin C on the neck. In a 6-month double-blind study in women with sun-aged skin on the low neck and arms, a 5% vitamin C cream improved overall scores and skin surface texture more than the plain cream (Humbert and colleagues, 2003).
Realistic Timelines
- Same day: film-forming tighteners such as Pepha-Tight give a tighter feel within minutes that lasts a few hours.
- 4 to 8 weeks: peptide studies above measured changes at 56 days to 2 months.
- 3 to 6 months: retinol and vitamin C studies ran 24 weeks to 6 months.
Deep folds from bone and fat changes will not disappear with a cream. If you want structural change, talk with a board-certified dermatologist about in-office options.
How to Choose a Neck and Jowl Product
- Look for named actives with stated percentages, not a long list of extracts with no amounts.
- Pair two types: a film-forming tightener for the look of lift today, and a peptide for the look of firmness over weeks.
- Pick a texture you will use twice a day.
- Sensitive skin: patch test on the inner arm for 24 hours and start at the lowest rate of each ingredient's range, then work up.

Frequently Asked Questions
What causes jowls and a sagging neck?
Several things at once: bone loss along the jaw, fat pads that thin and shift, less collagen and elastin, years of sun, weight changes and genetics. Skincare works on the skin layer, so it helps the look of firmness but not the bone and fat changes underneath.
At what age do jowls start?
Most people notice them in their 40s or 50s, though family traits and weight loss can bring them on earlier. Daily sunscreen on the neck and a firming routine started early help the area look firmer for longer.
Can you firm jowls without surgery?
You can improve how firm and defined the jaw and neck look with peptides, film-forming tighteners, retinol and daily sunscreen. Skincare will not lift tissue the way surgery does, so for marked sagging, a board-certified dermatologist can explain in-office options.
Does retinol help jowls?
Retinol helps the look of fine lines and crepey texture on the jaw and neck. In a 24-week study, 0.4% retinol improved fine wrinkling more than a plain lotion. It does not change bone or fat, so think of it as a texture ingredient.
Do face exercises work for jowls?
There is early evidence. In a small study, 16 women aged 40 to 65 who did 30-minute facial exercises for 20 weeks were rated by blinded dermatologists as having fuller cheeks, and their estimated age fell from 50.8 to 48.1 years (Alam and colleagues, 2018). It is one small study, so see it as promising rather than proven.
Can a cream tighten a sagging neck?
A film former such as Pepha-Tight gives an instant tighter feel, and peptides improve the look of firmness over 4 to 8 weeks of twice-daily use. Results are visible but modest next to in-office procedures.
